Quick facts

Kentucky State Police Post 13 reported a three-vehicle collision on KY-15 in the Watts community of Breathitt County on Wednesday, Apr. 22, 2026. Troopers said they were notified at approximately 11:13 a.m.

KSP reported two people were pronounced deceased at the scene, and that the crash involved a northbound commercial motor vehicle after an initial head-on collision between two passenger cars.

What has been publicly reported about how it happened

According to KSP’s preliminary investigation, a southbound Dodge Avenger crossed the center line into the northbound lane and collided head-on with a northbound Toyota Camry. KSP reported the impact pushed the Camry into the path of a northbound commercial motor vehicle, resulting in a secondary collision.

KSP said the crash remains under investigation and is being reconstructed.

Why this crash matters on KY-15

KY-15 is a key regional route through Eastern Kentucky. When a commercial motor vehicle becomes part of a multi-vehicle crash sequence—especially after a centerline crossover—investigators often focus on impact angles, time-distance factors, and whether commercial-vehicle data exists that helps confirm speed and braking.

Who may be liable

Public reporting provides a preliminary sequence, but liability depends on evidence and official findings. In a collision like this, responsibility questions may involve:

  • the actions leading to the centerline crossing,
  • whether any additional factors contributed to the secondary commercial-vehicle impact (visibility, traffic flow, reaction time),
  • and (if applicable) commercial-vehicle compliance and documentation—only if later evidence supports an independent issue.

Evidence that often matters in CMV-involved multi-vehicle crashes

In crashes involving commercial motor vehicles, key evidence can include:

  • scene reconstruction (impact points, lane positions, sightlines),
  • any available dash-cam or third-party video,
  • commercial vehicle telematics/ECM data where available (speed/braking events),
  • 911/CAD timelines and first-response documentation,
  • and roadway-condition context (traffic flow, weather, surface conditions).
